Welcome to the Kelly's Kidz application period for Children's Special Health Care (CSHC) premiums.
To be eligible, you must meet the following requirements, your child must:
You need to complete all the items listed below in order to be considered. Not everyone that applies will receive the funds as our distribution monies are limited. Below are three (3) SEPARATE pages, please print and read them all. You MUST provide proof that you are currently approved for CSHC, either by copying your eligibilty letter or copying your MI-Health card. Your child must be re-newing during the time frame of May 1 and October 31, 2010.

- Be a patient of Dr. Susan Youngs
- Already approved for CSHC and have a renewal date between May 1st and October 31, 2010
