The Kelly's Kidz Children's Special Health Care Grant application period is officially open. Please read the following instructions carefully to apply for this generous grant. Should you have any questions about this grant, please contact Kelly's Kidz today.

INSTRUCTIONS

Before downloading the application, please make sure you meet the following criteria: 

• Your child must have a physical impairment
• Your child must currently have Children's Special Health Care or be in the process of getting approved
• If you are not a patient of Oakwood's Center for Exceptional Families, you will need to download Oakwood's Medical Release form
• You have NOT received a grant for CSHC from Kelly's Kidz before
• You are willing to share your story (including photos) about your experience with Children's Special Health Care
